**** Enclose a copy of the parent(s)/legal guardian's 2011 (or 2010) Federal Income Tax Return which documents the adjusted gross
income or the application will be considered incomplete unless parents are not required to file a tax return. If a student's parents
were divorced or separated at the time of application and the parents filed jointly for the 2011 (or 2010) tax year, a copy of the W-2
form(s) for the parent with whom the student is living must be attached with a copy of the first page of the joint income tax return.
For students who qualify for free or reduced lunch, families may submit the qualifing letter from the school instead of tax forms.
