*** A copy of the first page of the parent(s)/legal guardian's Federal Income Tax Return from the previous year which documents the Adjusted Gross Income MUST be attached to this Financial Aid Form or this form will be considered incomplete. If a student's parents were divorced or separated at the time of application and the parents filed jointly for the previous tax year, a copy of the W-2 form(s) for the parent with whom the student is living MUST be attached with a copy of the first page of the joint income tax return.
