Parent’s checklist – Please complete the steps below:

1. Print out “YES Health Form” and “YES Parent Signature Form”, compete the forms, and send them to MITY at:  1600 Grand Ave., St. Paul, MN  55105 or use the addressed envelope that came with your acceptance letter or fax: 651-696-6592.  These forms must be turned in before the student attends classes.

5. If you child will be absent please call the Institute office, 651-696-6590, and leave a message on the voice mail.  Remember to include your child’s name and grade.  If you know your child will not attend an upcoming Saturday please fill out the form below and give it to the person at the registration desk.
